IBW client Samuel U. Rodgers Health Center recently earned several Community Health Quality Recognition (CHQR) Badges from the Health Resources and Services Administration (HRSA).
These CHQR badges recognize health centers that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response for the most recent Uniform Data System (UDS) reporting period.
Sam Rodgers has been recognized as a silver tier Health Quality Leader – its clinics have achieved the best overall clinical quality measure performance and its scores put it in the top 11-20% of all Federally Qualified Health Centers (FQHC) in the nation.
Sam Rodgers earned an Advancing Health Information Technology (HIT) Quality award, meeting all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health data to increase access to care and advance quality of care.
Sam Rodgers earned an Addressing Social Risk Factors Award. Which recognizes health centers screening for social risk factors impacting patient health and are increasing access to enabling services.
Sam Rodgers was also recognized as a Patient Centered Medical Home (PCMH), an approach that delivers high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system.
